Login to Your Provider Portal
Due to our overwhelming participation with providers joining our NEW Provider Portal, we will close the old Provider Portal effective 8/1/2024.

Important note: For each Tax ID number, the Local Administrator or designated user manager must be registered first. If you are not the Local Administrator or the person managing users for your group, kindly contact your Administrator for access or have them register for the portal initially.

Network Assignments
Add a Provider or Location
If you are a current Network Provider and need to add an additional provider or location to your current agreement, please complete the Provider Add form and submit to the Provider Relations Team at: chp.providernetwork@christushealth.org. If you currently have a USFHP agreement, please also include the required Background Check form with the submission.

Credentialing Status
Current Network - Added Provider
If you are a current network provider and need the credentialing status of a recently added provider to an existing agreement, please send an inquiry to christus.hp.credentialing@christushealth.org.
New Agreement Status
If you have a newly signed agreement that is in credentialing and are looking for the credentialing status, please send an inquiry to christus.hp.credentialing@christushealth.org Please allow 60 days from the date of agreement signed to allow for normal processing.
